To help make your experience with Star Ridge Stables smooth and well-organized, we’ve provided easy access to all required forms. Whether you’re signing up for lessons, registering for camp, or booking a birthday party, you’ll find everything you need below.
Please complete and submit all applicable forms before your first session, event, or program start date.

Available Forms

General Riding Waiver & Release Form

All students must complete this liability waiver before participating in any riding lessons or horse-handling activities. This ensures safety, accountability, and legal compliance for all parties.


Emergency Contact & Medical Information Forms

This form is required for minors and optional for adults. It provides vital details in case of medical needs or emergencies during lessons, camps, or special events.


Summer Camp Registration Packet

For summer day and overnight camp attendees. Includes camper details, parental permissions, emergency contacts, packing list acknowledgment, and behavior policy.


Pony Birthday Party Agreement

Outlines the schedule, rules, and safety expectations for private party bookings. Also includes required waivers for all child participants.


Horse Leasing Agreement

For approved riders entering a lease arrangement. Covers riding schedules, responsibilities, horse care expectations, and payment terms. Must be completed before lease start.


How to Access and Submit Forms

Forms are available by email or hard copy—please contact our office to request the specific forms you need.


Completed forms can be submitted:

  • By email to StarRidgeStables@gmail.com
  • In person at our Selma facility
  • Or mailed to: 726 WC Braswell Rd, Selma, NC 27576


All required forms must be submitted at least 48 hours prior to your first session or event.


Our team at Star Ridge Stables includes a USHJA-certified instructor with over 20 years of teaching experience, a licensed USEF "r" Judge in Hunter and Hunt Seat Equitation, and someone certified in Technical Large Animal Emergency Rescue.
